Before you decide to start your program, here are a few steps to follow:
- Contact us at 863-758-2273
- Check if we are in-network with your insurance plan. If not, we still accept other insurances as out-of-network. It’s your responsibility to check if the services provided are covered by your insurance. If no out-of-network benefits, you will need to self-pay.
- Check lab coverage with your insurance prior to the first visit. We are not responsible for lack of coverage.
- Send us a copy of ID and insurance card (front and back).
- Contact your local OBGYN and Reproductive endocrinologist, and request medical records to be sent to us. This is important prior to first visit. It will let us know if we can offer what you are looking for, so you don’t waste time and money. Review of documents and previous medical records doesn’t guarantee we can offer a consultation.
- Create an inpatient portal account (an invitation will be sent)
- Schedule your first appointment.
- Fill out clinical questionnaire online.
- Read and sign forms and consents prior to your first visit.
- Please be aware soon, out of state patients are required to be seen in the office at least the first visit, then subsequent visits can be provided through telemedicine (check with insurance if telemedicine is a covered service).
