Before you decide to apply to be a patient of our practice, please understand the steps to onboarding.
- We are unable to accept patients who are currently pregnant.
- ALL prospective and current patients must refrain from following the advice or suggestions from current or former patients on social media. DO NOT have any blood work, testing/procedures, or consultations performed until you see Dr. Jubiz for your consultation. Your treatment plan will factor in your past medical history and testing that is TIME SENSITIVE. The doctor has the right to refuse any results completed before your initial consultation and will require testing to be repeated.
Non-compliance regarding pre-testing before your consultation will factor in your being accepted as a patient with our practice. - Contact your insurance company to confirm if we are in-network with your insurance plan. We accept most AETNA, CIGNA, and BCBS plans – but not all. Please contact your insurance to confirm you have coverage to see Dr. Jubiz via telemedicine or in person in the State of Florida. CPT Codes: New Patient Consultation 99205, Follow-up Consultation 99215 - If you have an insurance carrier and we are out-of-network your plan, you will be considered self-pay. For self-pay patients, payment for your consultation fee must be received before 6pm EST the day before your consultation. Following your consultation, we can provide you with an itemized receipt that you can use to submit a claim to your insurance for reimbursement under your out-of-network benefits. Please check with your insurance plan for information regarding your out-of-network benefits.
- You must complete the information request form below to receive our onboarding letter and intake documents. You will receive a response within 1-2 business days. If you do not, please contact the office at: reception@jubizreproductiveimmunology.com
- The onboarding letter will consist of two PDF attachments which need to be completed and returned to our office with clear copies of your driver’s license and the FRONT AND BACK of your insurance card, regardless of whether your plan is in and out of network. If you have a secondary insurance plan, we need a copy of that insurance card (front and back) as well.
- Contact your OBGYN, RE (IVF), and any other specialist you have seen for treatment related to your reproductive health to have your complete medical records emailed or faxed to our office. (Release of Records forms can be found in the Patient Resources tab on our website).
- The submission of your intake documents and medical records is not a guarantee of acceptance as a patient in the practice.
- Once your intake documents and complete medical records have been received, Dr. Jubiz will review your file. This review can take 5-7 business days. Review of intake documents and medical records is not a guarantee you will be approved to schedule a consultation. This step is for Dr. Jubiz to determine if you would be a good candidate to proceed with a consultation. Consideration is dependent on several different factors, including your past medical history, treatment readiness, potential for adherence to treatment protocols, and whether our office culture is a good fit for your healthcare needs.
- If you are approved to schedule a New Patient Consultation, our staff will reach out to you with the next steps. If we decline to move forward with scheduling you for a consultation, we will provide you with a list of recommended providers who also specialize in Reproductive Immunology.
- While many of our consultations are performed via telehealth, travel to our Orlando office for consultation or treatment may be required.
